Moving towards Integrated health and care
David Thompson will share how the two trusts combined their resources to overcome capacity constraints, improving access and speeding up diagnoses for conditions such as cancer and heart disease, which ultimately enhanced diagnostic capacity. The next presentation will explain how information is used to create a person-centred care plan, which is being shared with professionals across health, social care, public health, and wider services to use at the point of care.
