Integrated Care case study session

This session brings together national and local case studies to explore how digital transformation is enabling more integrated, efficient care. It highlights the role of National Digital Shared Decision Making Councils in shaping priorities around AI, clinical safety and workforce development, alongside real-world examples from Somerset’s eSTEP, Leeds’ city-wide shared care plan, and Bolton’s rollout of Sunrise EPR across acute and community services. Together, these initiatives demonstrate how shared records, digitised workflows and cross-sector collaboration can improve patient outcomes, streamline services and support a seamless, end-to-end care journey across the NHS.
