Digitally driven Population Health Management

This session brings together system leaders to explore how data is being translated into real-world impact for population health. Greater Manchester will share how its Secure Data Environment is enabling trusted, collaborative use of data to drive research, innovation and service redesign at scale. Kent and Medway will demonstrate a practical Population Health Management approach, using segmentation, risk stratification and targeted interventions to support people with complex needs. Together, the session highlights how partnerships, trust and actionable insight are key to delivering more proactive, personalised care and improving outcomes across diverse populations.
