Empowering patients through technology

NHS Blood and Transplant share a case study on empowering patients through technology through the Serious Hazards of Transfusion (SHOT) scheme. As the NHS experiences unprecedented pressures, NHS England, UCLPartners, NHS North East London Integrated Care System, and Health Navigator have launched an ambitious programme that identifies people likely to experience extended hospital stays in the next six months. The programme will prevent crises before they occur and support people to take control of their health and stay out of hospital.
