Empowering patients through technology

This session explores how digital innovation is empowering patients, preventing avoidable harm, and reducing pressure on urgent and emergency care. From AI-enabled population health approaches that proactively identify and support high-risk individuals, to co-designed digital tools improving self-management in asthma and transfusion care, speakers will share real-world evidence and lessons from implementation. With a focus on engagement, equity, and outcomes, the session will highlight how technology can support safer, more personalised care, while addressing health inequalities and enabling patients to take greater control of their health.
