Integrated Care case study session

Hear how Leeds Community Healthcare NHS Trust has implemented a single care plan across Leeds city to support its home first/active recovery approach which ensures a more streamlined and efficient service delivery model, whilst also decreasing patient recovery times and increasing patient satisfaction. Session attendees will learn how Bolton NHS FT has extended the rollout of their EPR from an acute setting to community services. It marks a major digitisation to support care beyond the hospital by joining up patient records across settings. NHS Somerset ICB will share their journey to implementing an electronic Somerset Treatment Escalation Plan (eSTEP) in the SIDeR+ shared care record, replacing previous paper treatment escalation plans.
