Integrated Care case study sessions: patient-centred innovation

This session brings together three integrated care case studies demonstrating how shared digital care planning is improving patient experience, system collaboration and clinical decision-making. From end-of-life care planning across Greater Manchester, to recovery care plans integrated with Leeds Care Record, and neighbourhood-based health and social care transformation in Leicestershire, speakers will showcase practical approaches to delivering patient-centred innovation at scale. The discussion will explore how shared records, cross-sector partnerships and local ownership are enabling safer discharges, reducing duplication, and embedding meaningful change across integrated care systems.
