Join this session to learn about four different NHS case studies utilising analytics and data insights to enhance patient care. UCLPartners have developed CVDACTION, a smart data tool helping GP practices transform the prevention of cardiovascular disease. At Calderdale ICB, they have developed a risk prediction model allowing for early identification of high risk of dying in the next 1 year enabling better care and reduced system costs. Similarly, Cheshire and Merseyside are implementing a strategy to use population health analytics to be proactive; and identify the most complex patients. The East Midlands Imaging Network (EMRAD) will demonstrate the connection between harnessing data, enabling data access within governance confines, and driving innovation sustainably in the NHS.
