Delivering neighbourhood health teams
The NHS 10 year health plan includes the creation of a Neighbourhood Health Service, which aim to bring care closer to home and reduce healthcare inequalities. Neighbourhood health teams will bring together multi-disciplinary professionals, including GPs, community nurses, mental health specialists and social care staff, to focus on patients with multiple long-term conditions and people with complex needs.

Unplugged host Jordan Sollof and Dr Harpreet Sood, will discuss the importance of digital in supporting voluntary, community, and social enterprise organisations (VCSEs) and integrating them into neighbourhood care. As a practising GP in London, Dr Sood will share his perspective on what neighbourhood health means for primary care.
