Shared care records case studies

Learn lessons from three NHS case studies on different approaches to developing shared care records and the impact to date. The Leicester, Leicestershire and Rutland Care Record programme highlights the importance of human-centred design, good supplier relationships, collaboration and working to agreed regional data standards. Thames Valley and Surrey will explore the four key areas of partnerships, innovation, efficiency gains and sustainability. Integrated care planning in the GM Care Record is creating a more connected approach that places patients at the centre of their care journey.
